The subject of this thesis is about a study of the psychiatric assistance in brazil, psychiatry ans institutions psychiatrics depend deeply of the cultural and historical context where they develop. The evolution of both psychiatry and psychiatrics assistance in brazil should also deeply depend of the brazilian cultural and historical context. But, it seems that it is not the case at the level of the brazilian psychiatry, meanwhile the brazilian psychiatrics assistance is the reflection of the brazilian cultural context. The verification of this paradox determined our choice of the subject. Our researches were guide by the etablishment of two importants events : 1) the psychiatrics theories were imported; 2) the characteristics of the brazilian society instead of having been taken by the brazilians psychiatrists as being part and parcel of our culture, they had been taken as a backward with regard to the european civilization. Brazil is a country where the interpenetration of the three civilizations: the portuguese, the native ans the african, took a great place in the formation of the brazilian society, which is full of contrasts, syncretism and regionalisms. The mixing of races and cultures are even the support of the originality of the brazilian culture. All this context did not influence the psychiatry on its characteristics. The psychiatry, as a section of the medicine, was transplanted and grafted on the whole of the brazilian medicine. Psychiatry did not suffert any acclimatization and adaptation; it remained french or germanic as it arrived in brazil, meanwhile the psychiatric assistance suffered all the influence of the brazilian cultural context, from which we conclude that there was a divergence between the theories and the therapies imported and the practice. We tried to prouve all this by doing our research considering the three most important centers of psychiatry in brazil, as follows : rio de janeiro, sao paulo and recife. We have analysed the history and the evolution of their psychiatry and their assistance since their birth, at the nineteen century, till 1970.

Le theme de la these concerne l'etude de l'assistance psychiatrique au bresil. La psychiatrie et les institutions psychiatriques dependent du contexte historique et culturel ou elles se developpent. L'evolution de la psychiatrie et des institutions psychiatriques au bresil aurait donc du, elles aussi, dependre du contexte culturel bresilien. Or, ce n'est pas le cas au niveau de la psychiatrie, alors que l'assistance psychiatrique bresilienne, elle, n'est que le reflet du contexte culturel bresilien. C'est la constatation de ce paradoxe qui nous a fait pencher sur l'histoire de la psychiatrie au bresil, pour essayer de degager les facteurs qui sont a l'origine de ce phenomene. Nos recherches ont ete guides par la constatation de deux evenements importants : 1) il s'agissait d'une culture psychiatrique d'importation; 2) les specificites de la societe bresilienne au lieu d'etre prises par les psychiatres bresiliens comme faisant partie de notre culture, furent prises comme etant un retard par rapport a la civilisation europeenne et qu'il fallait le depasser. Le bresil est un pays ou l'interpenetration des trois civilisations : la portugaise, l'indigene et l'africaine donnerent naissance a la formation d'une societe tres particuliere, pleine de contrastes, de syncretismes et de regionalismes. Le melange des races et de cultures est a la base meme de l'originalite de la culture bresilienne. Ce contexte la n'a pas influence la psychiatrie dans sa specificite, c'est-a-dire que comme branche de la medecine scientifique, la psychiatrie a ete transplante et greffe au bresil, dans l'ensemble de ce qui etait la medecine bresilienne. La psychiatrie n'a subit aucune acclimatation ou adaptation; elle resta francaise ou germanique, telle qu'elle etait arrive au bresil. Cela n'etant pas le cas de l'assistance psychiatrique dont l'evolution fut determine par le contexte culturel bresilien, d'ou l'ecart existant entre les theories et therapies importes et la pratique. Les psychiatres bresiliens n'ont ainsi ni su et ni pu theoriser leur pratique effective. C'est ce que nous avons essaye de prouve en tenant en compte trois centres importants de la culture psychiatrique au bresil, a savoir : rio de janeiro, sao paulo et recife, en analysant l'histoire de l'evolution de leur psychiatrie et de leur assistance psychiatrique depuis leur naissance au xixeme siecle jusqu'aux annees 70.
