La psyche egaree - suivi longitudinal de 26 sujets ages, dements de type "alzheimer"

In this four-year study, we concentrated on alzheimer's disease, from a psychoanalytic reading of the processus, helped by interviews of psychoanalytic inspiration. The originality of the search lies in the detection of clinical signs typical of this pathology in a prospect both synchronic and diachronic. Therefore we discovered a graphic figuration of the man, absolutely singular with particularly the presence of a trunk that opens as the disease evolves. Another originality concerns the hypotheses. They convey two major ideas : 1) the alzheimer's disease presents itself like a unic example of dementalization to use the phrase of g. Le goues and p. Marty. 2) dementalization follows the model of a retrogenesis but up to a certain degree. The concepts of dementalization and of retrogenesis are studied with the proving of three steps in the disease : a first step or premisses of dementia during which an alteration in the conscious self can be observed ; a second step or real dementia that evokes a clinica observation of the limits of the self ; finally a third step or advanced dementia that is characterized by a psyche soma dismantle. In other respects, the thesis contains some views on the ways of accompanying the patients and their limits.

Il s'agit d'une etude longitudinale de la maladie d'alzheimer, a partir d'une lecture psychanalytique du processus, soutenue par des entretiens d'inspiration analytique. L'originalite de la recherche repose surle reperage de signes cliniques specifiques a cette pathologie dans une perspective a la fois synchronique et diachronique. Ainsi, nous avons decouvert chez ces patients une figuration graphique du bonhomme tout a fait singuliere avec notamment la presence d'un tronc qui s'ouvre au fur et a mesure qu'evolue la maladie. Une autre originalite tient aux hypotheses qui s'ordonnent autour de deux idees principales : 1) la maladie d'alzheimer se presente comme un exemple unique de dementalisation pour reprendre l'expression de p. Marty et g. Le goues, 2) cette dementalisation evoque le modele d'une retrogenese, mais jusqu'a un certain point. Les concepts de dementalisation et deretrogenese sont etudies a partir de la mise en evidence de trois etapes de la maladie : une premiere phase ou premices de la demence au cours de laquelle on observe une alteration du moi conscient, une deuxieme phase ou demence installee qui suggere une clinique des limites du moi, enfin une troisieme phase ou demence avancee qui se caracterise par un demantelement psyche soma. Par ailleurs, une reflexion est conduite sur l'accompagnement de ces patients et ses limites.
