Pain and suffering are distinguished theoretically and clinically, as an emotion and as an affect. The emotional pain is a clinical fact full of complexity revealing a triple function : of survival by screaming for parental care; of hysterical control of the bodies of the two parents; of avoiding the black melancholic hole. This thesis is illustrated by the case of a young man seriously burnt when five years old and actually presenting alcoholic and suicidary problemetics, between hysteria and melancholy. The emotional pain requires from the clinician a deepened formation between flexible, adaptative and opened psychotherapy to methodological modifications, and an analytical rigour in the tracking down of the unconscious "pulsio-fantasies".

Douleur et souffrance sont distinguees theoriquement et cliniquement, comme emotion et affect. La douleur emotionnelle est un fait clinique complexe qui revele une triple fonction : de survie, par appel aux soins parentaux; de controle hysterique du corps des deux parents; d'evitement du trou noir melancolique. Cette these est illustree par le cas d'un jeune homme gravement brule a 5 ans, qui presente une problematique addictive alcoolique et suicidaire, entre hysterie et melancolie. La douleur emotionnelle exige du clinicien une formation approfondie entre une psychotherapie souple, capable d'adaptations et d'ouverture methodologique, et une rigueur analytique dans le depistage des pulsio-fantasmes inconscients.
