The medical imaging field is called upon today to respond on the one hand to the rising needs of conventional radiology and on the other hand to perceive the medical field advances from an efficiency standpoint. Technological progress led to the development of PACS (picture archiving and communications systems) medical imaging networks. This research work investigates three fundamental question : do PACS networks contribute to the improvement of management and decision-making tools in the medical imaging field? The first historical approach traces the emergence of PACS networks in the evolutionary process of the medical imaging field along with the analysis of the supply's answer to the demand and the competitive environment. A second economical approach establishes on the one hand a cost element through a comparative analysis of five economic studies of PACS networks and, on the other hand, a set of efficiency indicators both technical and clinical, weighted and quantified, obtained as a result of our inquiry and that represent necessary tools to the economic justification of PACS. The third organizational perspective investigates the conditions and repercussions of the installation and management of these networks. The key players involved in this innovation are identified and the necessary compromises for the establishment of PACS are analyzed

Le secteur de l'imagerie médicale est aujourd'hui appelé à répondre, d'une part, aux besoins croissants de la radiologie conventionnelle et, d'autre part, à concevoir les progrès de la médecine dans une optique d'efficience. Au confluent des progrès technologiques, se sont développés les réseaux d'imagerie médicale PACS (picture archiving and communications systems). Ce travail est guide par trois réflexions : les réseaux PACS œuvrent-ils dans le sens d'une amélioration des outils de gestion et d'aide à la décision dans le secteur de l'imagerie médicale? Sont-ils souhaitables et possibles sur le plan économique? Cette innovation transforme-t-elle les valeurs autour desquelles les acteurs de ce domaine se reconnaissent? La première approche, historique, constitue une genèse des PACS dans la dynamique d'évolution de l'imagerie médicale ou les facteurs d'émergence sont identifiés, la réponse de l'offre à la demande et son environnement concurrentiel examinés. A travers une deuxième approche économique, nous élaborons d'un côté un élément de coûts, établi grâce à une analyse comparative de cinq études économiques sur les PACS, et de l'autre une batterie d'indicateurs d'efficacité clinique et technique, pondérés et quantifiés, obtenus suite à notre enquête et qui sont autant d'outils nécessaires à la justification économique des PACS. Dans une troisième approche organisationnelle, nous évoquons les conditions et les conséquences de la mise en place et la gestion de ces réseaux. Les acteurs concernés par cette innovation sont identifiés et les compromis nécessaires à la réalisation des PACS analysés.
